March 16th,1981 Radioactive gas leak. Duane, Palo, Iowa, USA - A radioactive gas leak at the Duane Arnold Nuclear Plant near Palo, Iowa went unreported. The U.S, Nuclear Regulatory Commission will issue a citation to operators, claiming the report should have been made to NRC Officials in Washington within an hour of the incident.
